• Student Rate - $10
• Basic - $35
• Professional - $75
• Silver - $500
• Gold - $1,000
• Platinum - $2,500
• Special Rate - $5

*The Special Rate is for individuals with a brain injury or family members with limited resources. The $5 membership fee covers processing the application and mailing materials.
