Emergency Fund
We're helping you today for tomorrow.
The Brain Injury Association of Arizona's (BIAAZ) Emergency Fund assists people
with brain injuries and their families with limited financial support. Currently, we can
grant a maximum of $250 per person per year. You must demonstrate a need for a
service or be at risk for harm if the service is not obtained.
The Emergency Fund may assist with: . A past due amount for rent or mortgage expenses, including late fees
. Medications and co-pays
. A past due amount for utilities
. Emergency dental or medical supplies
. Assistive technology or durable medical equipment purchase or repairs
. Groceries
. Vision evaluations, devices, or aides
The Emergency Fund can't assist with: . Vehicle repairs
. Cable bills
. Medical procedures or surgery
. Travel expenses
. Routine dental services
. Utility deposits or installment
. Apartment or house deposits
. Routine eyeglasses or vision services
. Any service or item not related to the applicant's brain injury
To apply for Emergency Fund assistance:
1. Contact the BIAAZ to request an application, or download the application here.
2. Submit your application and copies of the required documents to:

3. We will notify you regarding your application acceptance and funding. We reserve
the right to deny applications based on policies and procedures of the program.
4. When possible, checks will be processed within one business day after the application
approval.
Required documents
. A copy of the past due notice or bill for service (with the exception of
purchase of groceries)
. Photo identification (driver's license, ID card)
. Proof of Arizona residency if identification is not from Arizona (a utility bill
with an Arizona address)
. Verification of brain injury or contact information from a caseworker or
another professional who can verify your injury
